If you have any X-Rays/ Records that need to be transferred to our clinic then please print, complete and return the Incoming Records Transfer Form to us ASAP via fax: 763-561-1211 or email: info@zanedentalmn.com so that we can initiate the process of records transfer with your previous dentist right away.
